Only individuals who identify as adoptees and are fully and independently licensed in the U.S. may complete submissions.

New providers: Complete the Submission & Payment Form. Note that new listings may take 1-2 weeks to be added.

For annual membership renewals: Complete the Annual Membership Renewal & Payment Form each year prior to your renewal date to maintain an uninterrupted profile listing. A reminder of your renewal date will be emailed one month prior to your listing expiration date.

To make changes to your current listing, updates to your contact information, or add/remove states of licensure: Complete the Requested Revisions & Payment Form. Note that updates may take 1-2 weeks to be completed.
